Thank you for selecting our office for your dental care. To ensure that we properly diagnose your dental needs, your first visit with us will include a comprehensive exam, all necessary x-rays, and in most cases, a dental cleaning.
Our clinical team will review the results of your exam and x-rays and discuss any necessary future treatment you may need. 1) Request Appointment
If you have not yet scheduled an appointment, you may do so by requesting an appointment online or by calling our office at (952) 447-3777.
2) Download and Print New Patient Registration
After you schedule your appointment, we encourage you to download and print our new patient forms, so you can fill them out at your convenience. You can either fax or mail to us or bring them with you to your first appointment.
